I am In Network with a select number of insurance companies listed below. This means you get the highest benefit level that your insurance offers when you work with me. If the company is not listed, it either means I am in the process of resigning from the panel or I am considered "Out of Network" for the panel as I believe that my services are worth more than what the company has offered their in network providers.

Should you not see your insurance company listed, please do not hesitate to call me directly, and I will help you to verify my provider panel membership. Sometimes, insurance companies have multiple branches and may be called by a different name. Other times, the website or company representative may list differently or may not be looking up my name correctly. To assist you in verifying insurance, please go to PATIENT FORMS - Insurance Paperwork and select Insurance Verification Script in .pdf or .doc format.

You may also ask your insurance carrier if they offer "Out of Network" benefits. If they do, then likely a per cent of the fee will be covered by your insurance company.
